Once an appointment is made, we will send you new patient paperwork to an email or cellular device to fill out prior to your visit. If you need assistance completing paperwork or you prefer printed paperwork, we are happy to accommodate you on the day of your appointment.

Payment Options
For your convenience, we accept cash, personal checks, debit cards, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office if your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

Cancellations
If you are unable to keep your dental appointment, please let our office know within 48 hours of your appointment time. We ask for advance notice so we may offer your appointment to other patients who may be available instead. Please note that a cancellation fee may apply if a patient does not show up to an appointment or does not give sufficient cancellation notice.
